Activate your account today to have an easy access to your personal health care information and medical provider online at www.yourhealthfile.com. By activating your account, you get to view your account information, fill out forms, read the summary and result of your last appointment, review the medications listed on your health record, keep track of your allergies, safely view your latest lab tests result without hassle and most importantly, you get to communicate with your practice via a secure message system.

To start using yourhealthfile.com, you first need to activate your account by clicking the “Activate your account here” button and then a form will appear prompting you to provide a Username, Password and your Date of Birth. Once done completing the form, click the “Activate Account” button and you can now sign in to your personal account to review it and enjoy all the other features available. A “Forgot Info” button is also available if, by any chance, you forgot your username or password while logging in.

Yourhealthfile User Agreement

The final step in completing your online registration with your health file is by clicking the “I accept” button. By registering and clicking that “I accept” button, you agree to the Terms of Use mandated by your health file and to be legally bound to them.
